We accept most major insurance plans.Your insurance plan is a contract between you and your insurance company.We will try our best to work with you and your insurance carrier; however, it is ultimately your responsibility to know your insurance benefits and what services are covered by your plan.Please contact your insurance company with specific questions regarding coverage for our services.If a referral is required from your primary care physician before seeing a specialist, you must have the referral from your primary care physician before your initial visit in order to be "covered" for the visit.Also you must keep track of the number of referrals you have left for follow-up visits.Failure to do so may result in you being responsible for any charges.
For those patients on allergen immunotherapy (shots), you may be responsible for the entire or fraction of the copayment cost, depending on your insurance coverage.Also, each "shot" visit, as well as preparation of the serum counts as "a referral." If there is any question as to how many referrals you have left, always double check!
Copayments and outstanding balances are due at the time of service.Failure to collect a copayment is considered a violation of your insurance contract.Patients are responsible for the appropriate deductible and co-insurance.We accept cash, checks, and all major credit cards.A fee of $25.00 will be charged for returned checks.Also, you must notify the office for appointments canceled without 24 hours notice.Failure to do so may result in a $25.00 fee.Missing your appointment without giving us advanced notice is unfair to other patients who could have been seen during that time slot.
If you are a private/self pay patient, we offer an initial consult discount and will work with you on a payment plan if so needed.Also, in these tight financial times we understand if payment is an issue.We can work with you on a monthly installment plan as long as you let us know of your situation.
